Candidate Referral Program Terms & Conditions
Effective Date: July 11th, 2025
By participating in the Fusion Healthcare Staffing. LLC (“Fusion,” “we,” “us,” or “our”) Candidate Referral Program (“Referral Program”), you (“Referring Provider,” “you”) agree to the following terms and conditions. These Terms & Conditions govern your participation in the Referral Program and your eligibility to receive referral bonus payments.
- Eligibility – Referring Providers
To be eligible for referral bonus payments, the referring individual must:
- be a current or former Fusion contracted provider in good standing;
- submit the referral prior to the referred candidate’s first contact with a Fusion recruiter; and
- maintain good standing with Fusion.
If a Referring Provider is terminated during an assignment or otherwise is deemed not in good standing, Referring Provider will forfeit eligibility for any pending or future referral bonus payments.
Referring Providers may submit referrals before their own assignment begins. However, bonus payments will only be issued if both the Referring Provider and the Referred Candidate are hired and meet all applicable eligibility criteria.
- Eligibility – Referred Candidates
To qualify as an eligible referral, the Referred Candidate:
- must not be a current or former Fusion contracted provider or have been placed in a permanent role by Fusion in the past;
- must not have engaged in a two-way interaction with any Fusion representative (i.e., via phone, email, text, or other communication channel) within the past thirty (30) calendar days as retroactive referrals do not qualify for Referral Program;
- must not have been previously referred to Fusion. If a provider has already been referred, regardless of who submitted the referral, subsequent referrals will be deemed ineligible for bonus payments; and

must,
- for locum tenens roles, be confirmed to an assignment and complete a minimum number of hours set forth in the Qualifying Referral Bonus payout structure below; or
- for permanent roles, begin employment on the scheduled start date and remains employed in the role for a minimum of one hundred eighty (180) calendar days.
- Eligibility –Qualifying Referral Bonus
To be eligible to receive a Qualifying Referral Bonus:
- all eligibility criteria must be satisfied;
- the referral is only considered valid for ninety (90) calendar days from the date of submission; and
- the Referred Candidate must accept a role (by receiving a confirmation letter for a locum tenens assignment or accepting an offer letter for a permanent role) within ninety (90) calendar days of the referral submission.
- Referral Bonus Payout Structure
Qualifying Referral Bonuses are paid as follows:

- Physician Candidates (MD & DO):
- Locum Tenens Assignments: Up to $4,500 total (cumulative) bonus payment, upon completion of minimum worked hours within twelve (12) months of the assignment start date.
- Permanent Placement: $1,500 upon completion of one hundred eighty (180) days in role.
- Advanced Practice Providers (“APP”) (includes but is not limited to, nurse practitioners, physician assistants, CRNAs, RNs, DDS/DMDs, and allied health professionals):
- Locum Tenens Assignments: Up to $1,000 total (cumulative) bonus payment, upon completion of minimum worked hours within twelve (12) months of the assignment start date.
- Permanent Placement: $750 upon completion of one hundred eighty (180) days in role.
- Premium Specialties: Bonus amounts may vary based on specialty and work type. Certain “premium specialties” with high market demand for locums providers carry higher payouts as specified in the payout structure. Examples of premium specialties include, but are not limited to, the following:
- Surgery / General Surgery
- Hematology
- Oncology
- Obstetrics & Gynecology
- Cardiology, Radiology
- Gastroenterology
- Neurology
- Urology
- Cumulative Payment Structure :
Qualifying Referral Bonus payment amounts are total cumulative payments, based on a Referred Candidate working the minimum required hours for a particular payment tier, within twelve (12) months of their assignment start date. Incremental payments will be made as the Referred Candidate reaches the minimum required hours for each payment tier.
Examples:
- Example 1: If a Referred Candidate is a Premium Specialist and works between 40-119 hours in the first 12 months of a locums assignment, the Referring Provider will be paid $500 once the Rereferred Candidate has worked the 40th hour.
- Example 2: If a Referred Candidate is a Premium Specialist and works between 120-249 hours in the first 12 months of a locums assignment, the Referring Provider will be paid (1) $500 once their referral has worked the 40th hour; and (2) an additional $1,250 once the Referred Candidate has worked the 120th hour, for a total cumulative Qualifying Referral Bonus payment of $1,750.
- Example 3: If a Referred Candidate is a Premium Specialist and works 250 or more hours in the first 12 months of a locums assignment, the Referring Provider will be paid (1) $500 once their referral has worked the 40th hour; (2) an additional $1,250 once Referred Candidate has worked the 120th hour; and(3) an additional $2,750 once the Referred Candidate has worked the 250th hour, for a total cumulative Qualifying Referral Bonus payment of $4,500.
- Qualifying Referral Bonus Payments
- Qualifying Referral Bonuses will only be paid when all eligibility requirements are met.

Qualifying Referral Bonus payments will be processed and paid within sixty (60) days of:
- for locum tenens, when the Referred Candidate has met the minimum worked hours); or
- for permanent placement, completion of the 180-calendar day employment period.
- All Qualifying Referral Bonus payments are subject to applicable state and federal taxes and withholdings.
- Additional Terms
- Fusion reserves the sole and exclusive right to determine Provider and/or Candidate eligibility, validate Candidate referrals, and approve or deny Qualifying Referral Bonus payments.
- Fusion may modify, suspend, or terminate the Candidate Referral Program at any time, in its sole discretion with or without notice.
- Fusion shall not be liable for any lost opportunity or expectation arising from Candidate Referral Program changes or termination.
