Highlights
Opportunity Highlights:
- Monday & Wednesday: Direct patient care (5–6 patients/day; includes travel within a 30–60-mile radius; 6 total patient contact hours).
- Tuesday:
- Morning: High-risk huddle (in-office presence required).
- Afternoon: Patient visits.
- Thursday & Friday Remote Work
- Remote administrative time
- Chart reviews
- 1:1 virtual meetings with the Nurse Practitioners and other team members
- Occasional on site meetings / appointment visits with APPs as needed
- Manage a patient panel of 150–170 individuals while leading 4 Nurse Practitioners
- Full-time providers carry an average panel of 275 patients
- Work alongside social workers, care managers, navigators, and community health partners
- Shared call, rotated weekly across all providers in the state (triage team takes first call)
- Dyad leadership partnership with your Market Manager
Compensation & Benefits:
- Base Salary: $260,000–$290,000, with $290K targeted for these roles
- Annual Bonus: $40,000–$60,000, based on productivity, quality metrics, and team performance
- Retention Bonus: $75,000, paid over three years
