- Immunizations/Screening: MMR & Varicella (2) vaccines or immune titers; two-step TB test within past 12 months or recent QuantiFERON; CXR if TB positive
- Documentation: Case logs for last 2 years and any special procedure training
- CME: Preeclampsia-Eclampsia CME at initial application and every 2 years
