Coverage: 24-hour call

Average callbacks: 2-6 hours/day

OR schedule: 3-4 cases/day (note: another section lists 3-4/week)

Clinic volume: ~20 patients/day

Support staff: 1 PA and MAs; facility also notes 2 permanent physicians

Required procedures: Ureteroscopy, laser lithotripsy, TURP, TURBT, flexible cystoscopy

Procedure volume: ~10-15 per week

Requirements:

Board status: BC or BE

License: Arizona license or IMLC LOQ; DEA required

Experience: Minimum 1 year

Credentialing: ~30 days
